Community Assists work with several referral agencies to help transfer your residents

Do you have Medi-Cal eligible residents?  Has their care level improved and no longer need to be in a Skilled Nursing Facility?

Let’s transition them to a lower level of care through the Medi-Cal Assisted Living Waiver Program.

Here is what we will need to ensure timely and safe transfers

• Face sheets (indicating the resident has resided in your facility for 60+ days)
• Medication list
• H&P/PT-OT notes
• Current physician report (602 form) with TB testing or Chest X-ray (current)

That’s all!  Our Healthcare Professionals will take care of the rest, with selected Care Coordination Agencies (CCA)
