One stop Care Management coordination for ALW Members

Community Assists can help you expand your agency’s ALW program, we understand the multiple steps it takes to have resident approved.

Provide us with a current Skilled Care Facility (SNF) Face sheet, and we will do the rest!

  • All documents signed/dated
  • RN- Assessment – completed
  • ISP – completed by RN
  • POA – for healthcare decision (if applicable)
  • Medi-Cal eligibility verified
  • Completed file – reviewed again to ensure accuracy

We will complete the entire process and forward you a completed Zip file.  All you will need to do is submit the file to the DHCS for review.

Additional services offered:

  • Once the Medi-Cal member is approved, we will complete the placement/transition process.
  • Qualified and trained RN- Case Manger that is familiar with the ALW program
  • Re-Assessment every six months or change in condition/change in Tier level
  • Work closely with your CCA – director/manager.
  • Work closely with SNF and accepted RCFE/ALF.
  • Reassure family members on the entire process
  • Monthly Medi-Cal eligibility verified, on each of your members.
  • Monthly Medi-Cal Billing
